Okey 101 is played with four players over multiple rounds. The aim of the game is to finish with as few points as possible. The player with the least points at the end of all rounds wins the game. Points are determined by the numbers on the remaining tiles (example: a red 3 = three points, a black 11 = 11 points). The game can end when there are no more draw-able tiles left in the stack or when one player completes their hand. The game consists of a certain number of rounds (example: 3, 5, 7, 9, or 11 rounds). Each round lasts until one player finishes their hand.

After determining a dealer among the players, the dealer distributes 21 tiles to each player and takes 22 for themselves. The remaining tiles are placed face down on the table with one tile facing up. This open tile determines the joker (OKEY tile) for the game. The game is played counterclockwise. The player who deals starts and discards a tile before drawing. The player to their right then plays. Each player, in turn, either draws a tile from the stack or picks up the last discarded tile. After picking up a tile, if a player's series total reaches 101, they can lay down their hand (placing their series on the table). When a player lays down their hand, they place their series on the table next to other series. If a player cannot lay a tile down, they discard a tile to pass their turn. The player whose turn it is must always complete their turn by laying a tile on the table, even if they lay down all their hand they must still discard their last tile onto the table.

To lay down, you need a minimum score of 101. To lay down, you must have 3 or 4 sets of same-number different-colored tiles (for example, black 5, red 5, and one blue 5) or sequential sets of same-colored numbers (for example, red 7, 8, 9). Each set must consist of a minimum of 3 tiles. To add tiles to existing laid down tiles, a player must reach a minimum of 101 points and lay down their hand. During the same turn, a player can lay down their hand and add to other already laid down sets. If a player picks up the last discarded tile, they must use that tile. If a player has not laid down their hand yet and picks up this tile, they must lay down their hand when doing so, and this picked tile must be used within one of the sets they lay down. The picked tile cannot remain in your hand. If a picked tile cannot be used to form a set or to lay down a hand, it is returned and a tile is drawn from the stack. There is no penalty for this mistake.

Another way to lay down is to collect at least five pairs of tiles. A pair consists of two identical tiles. If a player opens the game by going with pairs once, they cannot open a normal set again in that game. However, they can add tiles to the sets on the table already opened by other players. If all four players in the game open with pairs in the same game, that round is canceled, and a new game starts. No player receives penalty points in this game.
