My Health is a digital personal health space that allows the citizens of Catalonia to easily access their health information and interact remotely with the Catalan Health System. It also enables access to the health space of children and individuals for whom one is legally responsible.

Users can consult and download clinical reports, diagnoses, clinical analysis results, and tests. It also provides access to the current medication plan for direct pharmacy pick-up, request primary care appointments, and access various digital health services. Through the eConsultation service, health inquiries can be made to healthcare professionals, complete procedures, and submit documents.

Access to My Health can be done using a digital certificate or username and password, both cases also require the personal identification code (CIP) found on the individual health card (TSI).

At La Meva Salut, you can find information from your shared medical history resulting from healthcare received within Catalonia's healthcare system centers. 

The main services and information available include:

Details of your primary care team (assigned doctor and nurse).

Diagnoses and clinical reports (from emergencies, hospital admissions, and outpatient care) as well as results from tests and examinations, and medical communications regarding work leave.

Administered vaccines and immunizations.

Current medication plan.

Personal agenda listing scheduled tests and appointments in primary healthcare and hospitals (as this service is gradually added).

Information regarding waiting lists if you are awaiting surgery.

Advance healthcare directive document, if registered.

Option to express willingness to donate organs and tissues and print a personalized donor card.

eConsultation for non-urgent inquiries with healthcare professionals.

Pre-scheduled appointment service.

Request for a change of doctor.

Arrangement of appointments for international vaccinations.

Downloading clinically coded information for secure sharing via the Health Data for Sharing Blue Button.

Please note that available services may vary depending on the healthcare provider (hospitals, primary care centers, etc.) corresponding to your residence location. These functionalities are expanded as providers make them available.

Public utilization healthcare centers under Catalonia's Integrated Public Health System (SISCAT) are progressively communicating stored information. Therefore, it's possible that not all data from your medical history will be immediately available.
